Due to the current COVID-19 pandemic, patients are asked to register without coming into the practice. To register you will need to complete our secure online registration form.

Registering with the Practice

You can apply to register with us by completing one of the registration forms available at reception.

We never discriminate on the grounds of age, sex, medical condition, race, disability or social class, and if we are unable to register you we will always provide you with a reason in writing.

If you are already registered with us and move out of the area, we may still be able to provide you with medical services. Contact reception for more details.

IMPORTANT INFORMATION

Thank you for considering Lockwood GP Surgery to become your new General Practitioners.

Before registering it is important that we point out that the Doctors do not routinely or regularly prescribe the following types of medications:

  • Opioid medication
  • Sleeping tablets or Tranquilisers
  • Preparations such as Zopiclone, Diazepam, Nitrazepam, Temazepam or
  • other Benzodiazepines
  • Pregabalin
  • Gabapentin
  • Tramadol

The practice will offer a reduction programme for patients on medications such as these. The gradual reduction will take place over a specific period of time agreed by the practice and is medically recognised as safe and effective. By registering you will acknowledge that you wish to enrol in the reduction programme.

Named GP

Whilst you are registered with the practice, every patient has a named GP who is responsible for overseeing their clinical care, they are known as your ‘Usual GP’. If you are unsure who your ‘Usual GP’ is please ask.
